Job Overview
The Care Manager/Designated Care Manager is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in assisted living, long-term care, and reminiscence neighborhoods. The role includes hands‑on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP), while maintaining a safe and comfortable home‑like environment. The Care Manager demonstrates Sunrise Senior Living’s mission “to champion quality of life for all seniors.”
Responsibilities
Review, read, notate, and initial daily log to document resident information.
Participate in development of ISPs and monthly updates.
Maintain responsibility for assigned residents, checking on them throughout the shift.
Observe, report, and document changes in resident conditions (skin, behavior, alertness, weight, diet, activity).
Attend Cross‑Over meetings and notify supervisor / resident care director of any increase in care needs.
Respond to security systems and call bells promptly.
Greet guests, family, residents, and team members with courtesy.
Assist with continence management and disposal of supplies.
Communicate with families as a resource.
Help residents maintain independence and physical safety, promoting dignity.
Participate in and lead assistance with activities of daily living (ADLs) and encouragement (IEA) of residents.
Engage residents in life‑skills and enrichment activities, especially in reminiscence.
Maintain cleanliness, hygiene, and safety per standards.
Host and engage in resident activities daily.
Partner with community team to ensure compliance with national and provincial regulations.
Practice safety procedures, including PPE, fire safety, lockout/tagout.
Ensure proper housekeeping: cleaning rooms, making beds, laundry, and common areas.
Serve meals in dining room, assist with setup and cleanup, and observe dietary instructions.
Participate in reminiscence‑neighborhood specific duties (e.g., IEA residents, afternoon social).
Comply with training requirements and maintain certifications.
Attend team and training meetings as directed.
Qualifications
High School diploma/GED (required); college preferred.
CPR Certificate and First Aid as required.
Minimum 18 years of age.
Previous experience working with seniors preferred.
Desire to serve and care for seniors.
Ability to make decisions in resident’s best interest.

Compensation
The base pay range is $15.70/hr to $19.65/hr, with potential discretionary bonuses based on performance. Additional compensation may be awarded according to company plans.
