Overview Care Management & Professional Outreach role at Arosa. Work directly with clients and their family members; provide support for home care services and community-based care management services. Qualifications
Educational Minimum Requirement: a Bachelor’s Degree. Preferred candidate: Master’s Degree in a healthcare-related field.
Ability to interface effectively and professionally with client support, including family, physicians, attorneys, etc.
Ability to work in a team environment required.
Acceptable candidate: Bachelor’s or Master’s Degree in Social Work, Nursing, Gerontology, Psychology, Health and Human Services, Occupational Therapy, Licensure in nursing (LVN/LPN or RN), or other healthcare-related fields.
Preferred: A minimum of two (2) years experience in care management; experience working with the aging population preferred; other professional experience may substitute.
Excellent written and verbal communication skills required.
PC literacy required.
Valid driver’s license and proof of current auto insurance required.
