As a Part-Time Care Manager, you will work with youth in an early identification and intervention program who are at increased risk for the development of a psychotic disorder (based on family history and/or emergence of troubling symptoms).
Our Clinical High Risk Psychosis (CHRP) program is a community based program that provides services to consumers throughout Cape May, Atlantic, Salem, Cumberland, Gloucester, Burlington, and Ocean counties working with youth, adolescents and young adults, up to age 25 experiencing a crisis.
Base Location: Cherry Hill, NJ
Schedule: Part-time; 20 hours; Monday - Thursday; 2 days 9:00am-2:30pm; 2 evenings 2:30pm- 8:00pm
Responsibilities Assist Program Consumers and their families in linking with community resources as needed
Coordinating care for the consumers by collaborating with treatment providers, school, staff and other relevant individuals
Document all interactions with consumers, their families and providers and communicate with the team, maintain up-to-date information in the consumer’s electronic health record
Meet with consumers in their homes and community as needed in order to assist with educational, vocational and other needs
Participate in weekly Supervision with the Team Lead
Participate in Team Meetings and collaborating with treatment team
Promotes family-driven, youth-guided services and recovery-oriented environment; assists individuals and their families in making decisions, taking action and goal planning. Provides education and information to the individual and their family
Performs other related duties as necessary

Qualifications Bachelor’s Degree in Social Work or related field required;
Valid Driver's License in good standing.
