Community Name: Huntington Common Job ID: 2025-223515 Job Overview The Care Manager/Designated Care Manager is responsible for providing the highest quality care and services to residents and their families in assisted living/long-term care and reminiscence neighborhoods. The role involves hands-on care, physical and emotional support as outlined in each residents Individualized Service Plan (ISP), while maintaining a safe, home-like environment. The Care Manager demonstrates Sunrises Mission to champion quality of life for all seniors, in accordance with laws, regulations, and company standards.
Responsibilities & Qualifications Essential Duties Supporting our Mission, Principles of Service, and Core Values is fundamental. Responsibilities include but are not limited to:
Review and document resident information in Daily Log and participate in ISP development and updates.
Manage designated residents during shifts, monitor their well-being, and respond to their needs.
Observe, report, and document changes in residents conditions.
Attend daily care meetings and communicate any increased care needs or condition changes to supervisors.
Respond promptly to call bells and security alerts.
Greet and communicate professionally with residents, families, and team members.
Assist with activities of daily living, promote independence, dignity, and safety.
Engage residents in life skills, activities, and reminiscence programs.
Maintain safety and hygiene standards, participate in activities, and support risk management and safety protocols.
Additional Responsibilities Maintain cleanliness and sanitation in resident rooms and common areas.
Serve meals, assist with dining, and observe dietary changes.
Participate in team meetings, training, and engagement programs.
Adhere to safety procedures, report incidents, and ensure compliance with health regulations.
Qualifications High School diploma/GED, CPR and First Aid certifications as required.
At least 18 years old; experience with seniors preferred.
Desire to serve seniors, good judgment, communication skills, and ability to handle multiple priorities.
