Overview
Westchester Jewish Community Services (WJCS), a nonprofit founded in 1943, has grown to be the largest provider of licensed outpatient community mental health services and one of the largest human service agencies in Westchester County. We provide state‑of‑the‑art programs and services and compassionate care to more than 20,000 people of all ages and backgrounds each year.

We are seeking a full‑time Care Manager to work in our Certified Behavioral Health Clinic (CCBHC) in Yonkers, NY. This role will be part of a small multidisciplinary team that provides support and assistance to consumers (aged 4 throughout the lifespan). The salary for this full‑time position is $50,000.

Responsibilities

Enroll individuals into wrap‑around services, providing initial engagement and assessment for COPS referrals and new clients entering services.

Complete initial and ongoing NOMS assessments.

Conduct Social Determinants of Health assessments.

Collaborate with clients and clinicians to create care plans tailored to the individual.

Coordinate with care team and follow up on referrals.

Primary care management.

Participate in multidisciplinary care planning meetings and grand rounds.

Assist the team with crisis intervention and stabilization as needed to support the client.

Participate in outreach and recruitment activities with the team, forging relationships with community providers to help individuals access care.

Requirements

Bachelor’s degree in Social Work or related field.

Bilingual English/Spanish is preferred.

Minimum of 1 year of relevant experience.

Excellent interpersonal, engagement, and communication skills.
