Under the supervision of the Youth Services Program Manager, the Care Coordinator will provide comprehensive support services to at‑risk youth, including those involved in the juvenile justice system. The Care Coordinator delivers intensive 1:1 wraparound case management services to RAICES and CCP youth, including crisis intervention, counseling, advocacy, and mentorship. Additionally, this position provides guidance to Case Specialists and youth services staff by offering training and professional development resources. The Care Coordinator is responsible for identifying and addressing barriers faced by youth and staff, connecting them with appropriate services and referrals, and supporting the overall success of the Youth Services Department.

Essential Functions / Responsibilities

Provide leadership support to CCP and RAICES staff by facilitating case consultations, offering supervision, and delivering ongoing guidance to ensure effective service delivery and staff development.

Provide intensive 1:1 wraparound case management services to at-risk youth in RAICES and CCP programs, including crisis intervention, counseling, advocacy, mentorship, and referrals.

Actively support youth and families by participating in case conferences, court sessions, and other critical interventions.

Collaborate with RAICES Youth Engagement Counselors, attend school groups, workshops, and outreach events.

Conduct outreach and wellness calls to youth and families to assess needs and ensure ongoing support.

Assess youth needs and develop service plans to reduce the likelihood of escalation into higher levels of detention.

Provide and facilitate educational workshops, leadership activities, and community services for youth and staff.

Lead youth support groups in a safe and structured environment to build community and encourage positive change.

Implement behavior management strategies that promote positive youth development.

Collaborate with program staff to identify individual needs and create proactive strategies that foster social-emotional growth and success.

Provide case consultations and support to Case Specialists, ensuring holistic and trauma-informed approaches.

Maintain accurate program records and submit data in accordance with funding and reporting requirements.

Attend required team meetings (weekly, monthly, and/or quarterly). Coordinate services with other MNC programs and community partners to ensure smooth referrals and continuity of care.

Assist with planning and participating in MNC-sponsored community and cultural events.

POSITION QUALIFICATION REQUIREMENTS

Bachelor’s degree required in Counseling, Social Work, Psychology, or related field.

Experience

Minimum 2+ years of nonprofit experience.

Knowledge of trauma-informed, restorative, and harm-reduction practices.

Experience in a similar role supporting youth and program staff.

Experience with the juvenile justice system strongly preferred.

Requirements

Valid California Driver’s License, current auto insurance, and clean driving record.

Language

Bilingual/bicultural preferred.

Tools & Equipment

Standard office equipment (computer, phone, copier, filing systems, fax).

Confidentiality

Must maintain the highest level of confidentiality regarding client information.

Physical Demands

Sitting: ~50% of the time

Walking: ~30% of the time

Standing: ~20% of the time

Frequent hand/finger use (typing, handling, operating equipment)

Occasional lifting/moving of up to 30 lbs.

Occasional reaching with arms/hands

What We Offer

Pay Rate: $70,000 - $72,000 Annual Salary

Medical – Kaiser and CCHP / Dental – Principal / Vision – Vision Care / Life Insurance - $50,000 / 403b – 3% match / FSA
