Temporary Care Coordinator

Sentara Health Plans is hiring a Temporary Care Coordinator for the Northern/Central Virginia area!

Position Status: This temporary position is set to last a minimum of 3 months but has the possibility of extending.

Position Location: Hybrid in Northern/Central area. Must be able to travel 3-4 days a week for in-person assessments.

Area includes: Culpeper, Spotsylvania, Louisa, Rapphannock, Fredericksburg, Woodbridge, Prince Edward, Farmville, Manassas

Position Requires Both In Person Face-to-Face Assessments and Remote Telephonic Assessments! Applicants Must Be Located in Western/Roanoke Area

Standard Working Hours: 8:00AM to 5:00PM

Education

Must possess a degree in Health & Human Services or one of the following related fields: Art Therapy, Behavioral Sciences, Child Development, Cognitive Sciences, Community Mental Health Counseling (MH, Vocational, Pastoral, etc.), Counselor Education, Early Childhood Development, Educational Psychology, Gerontology, Healthcare Administration, Human Development, Human Services, Marriage and Family Therapy, Music Therapy, Nursing, Pharmacy, Psychiatric Rehabilitation, Psychology, Rehabilitation Counseling, Social Work, Sociology, Special Education, Speech Therapy, Therapeutic Recreation, Vocational Rehabilitation. All degrees must be from schools that are listed as accredited on the U.S. Department of Education College Accreditation database found on the U.S. Department of Education website.

Certification/Licensure

No specific certification or licensure requirements

Experience

1 year of Long Term Care and Health Plans experience required

Case Management/Care Coordination experience strongly preferred

Experience with Community Health Resources strongly preferred

Experience initiating care plans and both subjectively and objectively conducting assessments
