Responsibilities
Assist with training and directing staff to follow department workflows.
Act as team champion for workflows, providing training and workflow oversight to care coordinators.
Co‑lead monthly meetings with manager to ensure appropriate follow‑up is completed, including taking minutes; support or lead weekly check‑ins if manager unavailable.
Assign/provide coverage within department in case of team members’ absences.
Support manager to maintain team training manual.
Conduct in‑person site visits to assigned PCC locations for rounding with employees per frequency designated by manager.
Provide feedback on performance directly to employees as well as to manager; assist with performance evaluations.
Maintain equipment and supplies of team members; notify manager and/or site Practice Manager of needed items.
Promote teamwork and problem‑solve issues concerning patient care.
Attend external meetings with community partners and third‑party payers at the discretion of the Director or Care Coordination Manager.
Provide excellent customer service to internal and external customers.
Conduct all duties outlined in the Care Coordinator job description.
Decrease barriers to care, increase motivation, and foster open communication.
Contribute to patient education materials and strategies to support care coordination.
Work with manager and team to create flow charts, workflows and document‑tracking processes as needed.
Perform other duties as required and assigned.

Experience & Training
High school diploma required. Associate’s or Bachelor’s degree preferred and/or 2 years of related experience.
Previous healthcare experience preferred.
Demonstrated ability to effectively and efficiently handle a demanding workload involving multiple tasks.
Demonstrated skills in the designated and certified area of practice arena and the ability to work and collaborate on a health‑care team.
EMR experience preferred.
Physical Demands
Remain stationary 50% of the time.
Move around the clinic site 50% of the time.
Constantly operate a computer, computer printer, copy machine, and telephone.
Occasionally lift or move objects of 10 pounds from one level to another.
Transport between sites as needed.
Cover other shifts as necessary.
Personal Characteristics
Ability to read and write proficiently in English.
Bilingual English/Spanish preferred.
Excellent interpersonal skills.

Technical & Software Knowledge
Computer proficiency: PC, email, fax, computerized voicemail, common office machines.
Development of report criteria and quantitative skills to analyze basic data.
Experience with MS Office (Excel and Word) required.
