Care Coordinator/ Scheduler - Mind&Mobility
Location: Brandon, FL
Job Type: Part-time, in-office (Monday- Friday 9am-2pm)
On-call: 1–2 weekends per month (additional compensation provided)
Pay: $20-$21/hr

We're looking for an organized and compassionate Care Coordinator / Scheduler to join our team!
Position Summary:
You'll play a key role in ensuring quality care for our clients and a positive experience for our caregivers by coordinating daily operations and supporting both our staff and clients.
Key Responsibilities:
- Help retain quality caregivers and clients through excellent communication and staffing support
- Coordinate daily operations, maintaining communication with caregivers, clients, and agency personnel
- Set up and maintain new patient electronic files
- Conduct client intakes and assessments as requested, identifying client needs and presenting care options
- Assign and schedule caregivers in accordance with agency policies and compliance standards
- Maintain and update caregiver calendars and client schedules in AxisCare. Communicate care details and schedule changes promptly to all necessary parties
- Handle inquiry calls, follow up with leads, and convert inquiries into active clients
- Strengthen relationships with referral sources and proactively reach out to potential clients
- Verify timesheets for payroll and billing purposes and communicate billing inquiries as needed
- Participate in caregiver performance evaluations and help resolve client or caregiver concerns
- Provide coverage for other Care Coordinators when needed
- Recommend improvements to policies, procedures, and workflows
Qualifications:
- Previous experience in home care scheduling, coordination preferred
- Excellent organizational and communication skills
- Strong attention to detail and ability to multitask in a fast-paced environment
- Compassionate, professional, and client-focused demeanor
- Proficient with scheduling software (AxisCare experience a plus)
- Reliable transportation for local travel
