Job Title: Care Coordinator, RN

FLSA Status: Exempt

Sign-On Bonus: $3,000; no relocation package is currently offered
Location: Senior TLC Center – Charlotte, NC

Position Summary: The Care Coordinator serves as a key member of the Interdisciplinary Team (IDT), responsible for ensuring seamless coordination of care across clinic, home, hospital, and community settings. This role supports participants by facilitating communication among providers, monitoring care delivery, and ensuring that individualized care plans are implemented effectively. The Care Coordinator advocates for participant needs, promotes continuity of care, and helps navigate complex healthcare systems to optimize outcomes and quality of life.

Key Responsibilities:

  • Serve as the primary point of contact for assigned participants, ensuring timely coordination of services across care settings
  • Collaborate with the IDT to develop, implement, and update individualized care plans based on participant assessments and evolving needs
  • Monitor participant progress and communicate changes in condition to appropriate team members
  • Facilitate referrals, appointments, and transitions between care environments (e.g., hospital discharge, home care setup)
  • Support medication reconciliation and follow-up after hospitalizations or emergency visits
  • Advocate for participant preferences and ensure care plans reflect their goals and values
  • Educate participants and caregivers on health conditions, treatment plans, and available resources
  • Document all care coordination activities accurately and in compliance with regulatory standards
  • Participate in quality improvement initiatives and contribute to team-based problem solving
  • Attend IDT meetings, staff conferences, and care planning sessions
  • Assist with crisis intervention and support during emergency situations
  • Promote a culture of respect, compassion, and teamwork across all interactions

  • Strong clinical judgment and ability to manage complex care needs
  • Excellent communication and interpersonal skills for working with participants, families, and providers
  • Ability to prioritize tasks and manage multiple responsibilities efficiently
  • Knowledge of community resources and healthcare systems
  • Commitment to person-centered care and ethical practice

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in North Carolina
  • Graduation from an accredited nursing program
  • Basic Life Support (BLS) certification
  • At least one year of clinical nursing experience, preferably in geriatric or outpatient care
  • Solid understanding of nursing principles, patient care standards, and healthcare regulations

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN)
  • Experience in geriatric or senior-focused clinical settings
  • Advanced certifications (e.g., Geriatric Nursing Certification)
  • Familiarity with electronic health record (EHR) systems
  • Strong communication and interpersonal skills for working with elderly patients and families

Health Requirements:

  • Must be medically cleared for communicable diseases and up-to-date on immunizations
  • Must be able to provide care to adult and geriatric populations
