Find your purpose working as a Care Coordinator Registered Nurse with CentraCare! We are seeking a dedicated RN to join the team and make a meaningful impact on the lives of our patients and families. In this role, you will support patients in the West and Southwest Regions of CentraCare doing all work with patients over the phone.

You will serve as an advocate, helping individuals navigate the health care system and access the services they need to achieve optimal health and function within their environment.

Goals of Care Coordination at CentraCare Clinics:

• Develop proactive and anticipatory care plans that integrate input from multiple disciplines and service systems.
• Assist patients and families in accessing necessary services and resources.
• Facilitate effective communication among health care professionals.
• Promote cost efficiency by reducing service duplication and avoiding unnecessary expenses.
• Promote the physical and emotional well-being of patients.
• Support the quality of life for patients and their families.

Schedule: Full-time | 72 hours per two weeks | Hybrid position Primarily work from home with a few in person meetings quarterly Monday Friday | 8-hour shifts between 7:30 am 5:00 pm

Pay and Benefits: Starting pay begins at $32.66 per hour; exact wage determined by years of related experience Pay range: $32.66 $49.01 per hour Full-time benefits: medical, dental, PTO, retirement, employee discounts and more! Tuition reimbursement and college grant programs available.

Qualifications: Graduate of an accredited school of nursing Current licensure as a Registered Nurse in the State of MN Current American Heart Association (AHA) BLS Healthcare Provider card Experience with the diversity of the local community preferred Previous experience with care coordination preferred CentraCare has made a commitment to diversity in its workforce.
