Cleveland Clinic Care Coordinators play a vital role in helping patients actively manage their careimproving outcomes and reducing Emergency Department visits, inpatient stays and hospital readmissions. As a Care Coordinator supporting a new orthopedic practice, you will ensure coordinated, high-quality, patient-centered care across the entire orthopedic continuum. In this role, you will guide patients from preoperative optimization through discharge and recovery, leveraging your nursing expertise to deliver timely education, manage risk and facilitate seamless care transitions. A licensed nurse is essential for this position, as it requires sound clinical judgment, direct patient education, medication review and the ability to assess and escalate care when needed. Acting as the first line of clinical triage in the post-acute phase, you will serve as a crucial liaison between the surgical team, rehabilitation and primary carehelping to reduce complications, support adherence to evidence-based protocols, and promote quality, safety and patient advocacy at every step.
A caregiver in this position works varying hours between 7:30am to 5:00pm with most days being 8:00am to 4:30pm.
