Job Summary As a Care Coordinator in our Hospital Division, you will collaborate with Bergen New Bridge Medical Center Administration and treatment teams to enhance the client experience, assist with transitions to care, and improve community communication. You will be part of a dedicated team working towards reducing recidivism rates and collaborating with complex discharge plans to create client stability and increase access to community resources.
What You Will Do Engage individuals/families regarding assessment of treatment needs and transition to services, in collaboration with hospital staff.
Collaborate with hospital staff and community providers.
Provide training and education related to community and financial resources.
Assist with transition to treatment providers including directly facilitating linkages to services, warm hand-offs/service coordination, treatment enrollment assistance, and related activities.
Assist individuals with developing an effective support system to help them maintain stability in the community.
Coordinate care by documenting observations and information about the individual in the EHR and communicating insights to appropriate staff.
Participate in assessment and evaluation of individuals at intake/admissions, during ongoing recovery/service delivery and at discharge.
Collaborate with treatment providers to establish comprehensive, individual-centered linkage/referral recommendations and discharge plans.
Work with administrative staff to ensure all services are documented based on billing and regulatory standards; collect information on individuals and reflect all demographic data.
Participate in quality assurance activities and cross-functional action teams.
What We Are Looking For Bachelor\'s Degree in Social Work, Psychology, Counseling or related field; Master\'s preferred.
Knowledge or experience with providing case management and discharge planning
