Role Description
This is a full-time, on-site position located in Seattle, WA, for a Care Coordinator in Cancer Prevention. The Care Coordinator will be responsible for guiding patients through preventive care services, supporting case management efforts, and assisting with patient navigation. Daily responsibilities include scheduling appointments, providing education on preventive screenings, coordinating care among providers, maintaining accurate records, and ensuring patients access available resources. Customer interaction and collaboration with the clinical team are essential components of the role.

Qualifications

Strong skills in Care Coordination and Case Management to ensure seamless patient care

Effective Communication skills to provide clear education and support to patients and collaborate with team members

Experience in Home Care and Customer Service to prioritize patient needs and deliver compassionate assistance

Ability to work independently and maintain accurate, organized records

Bilingual proficiency in Spanish or other languages is highly preferred

Experience in healthcare, public health, or a related field is a plus

High school diploma or equivalent required; a bachelor’s degree in social work, public health, or a healthcare-related field is an advantage
