Care Coordinator – Strive Health Services LLC
Job Overview
Join Strive Health’s team as a Care Coordinator. You will serve as the primary point of contact for patients with chronic kidney disease (CKD) and end‑stage renal disease (ESRD), coordinating resources, ensuring continuity of care, and proactively following up to fill any gaps.
What You’ll Do
Perform outbound calls to patients and providers, scheduling appointments and following up on care.
Answer inbound calls from patients, providers, and other resources, and maintain clear communication.
Coordinate with clinical resources and providers to ensure a smooth continuum of care, assisting with application and paperwork for resources.
Monitor patient hospitalizations and follow up as necessary, confirming Strive gathers all relevant information.
Provide patients with education materials and send communications to primary care physicians, nephrologists, and specialists for new enrollments/appointments.
Collaborate seamlessly with all levels of a clinical team, partnering closely with the Strive Nurse Practitioner to manage all care related resources and appointments.
Provide in‑person patient care, which may include standing, sitting, walking, pushing, pulling, and lifting.

Minimum Qualifications
2+ years combined of related education, experience, or certification.
Current BLS or CPR certification required.
Efficient and reliable transportation, including an active driver’s license, to travel across an assigned region.
Preferred Qualifications
Active Community Health Worker (CHW) certification.
Customer service experience.
Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook.

Communicates clearly, respectfully, and thoughtfully.
Hourly Range
$22.25–$25.00
