Summary:
Are you a compassionate connector with a knack for organization and a heart for helping others? We’re looking for a Care Coordinator to be the welcoming face of our supportive housing program!
What You’ll Do:
• Meet new participants and help them feel at home
• Conduct interviews, verify eligibility, and build personalized care plans
• Link clients to vital resources like healthcare, benefits, and community support
• Collaborate with clinical teams and keep documentation on point
• Be a steady guide through life’s ups and downs

To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.
Qualifications:
• Bachelor’s degree in human services, psychology, or a related field, or a bachelor’s degree with 5 years of relevant experience
• Proficiency in Electronic Health Records (EHR) systems (e.g., Netsmart, Quantum)
• Knowledge of HIPAA, Medicaid/Medicare documentation standards
• Comfortable using Excel, Google Sheets, or databases to track caseloads
• Familiar with resource directories and referral platforms
• Able to manage multiple caseloads using tools like Microsoft Teams
• Understanding of treatment planning tools (IM+CANS, LOCUS, IATP)
• Familiarity with billing codes and service documentation requirements

Compensation & Benefits:
• Base Salary: $52,000 per year
• Paid Time Off: 11 vacation days, 12 holidays, 3 sick days, 3 personal days
• Illinois Paid Leave: 40 hours (accrued)
