What You’ll Do
The Care Coordinator works collaboratively with the care team to provide ongoing support and communication to chronic kidney disease (CKD) and End-Stage Renal Disease (ESRD) patients. Acts as a single point of contact to coordinate resources along the care delivery spectrum, identify gaps, and provide proactive follow-up. Reports to the Lead, Care Coordinator.
Performs outbound calls to patients to understand clinical needs and connect them with resources; performs outbound calls to providers to schedule or follow up on care. Answers inbound calls from patients, providers, and other resources.
Follows up with patients to ensure needs are met and schedules future check-ins. Notifies patients of location and appointment times as needed.
Coordinates with clinical resources and providers to ensure smooth continuum of care. Assists with completing applications for resources and paperwork for provider visits.
Monitors hospitalizations and follows up with care team members and outside resources to collect relevant patient information for Strive.
Provides patients with education materials and sends communications to primary care physicians, nephrologists, and specialists for new enrollments/appointments.
Collaborates with all levels of the clinical team and partners with Strive Nurse Practitioner to manage care resources, appointments, transitions, and gaps.
Provides in-person patient care as needed, which may include standing, sitting, walking, pushing, pulling, and lifting.
Minimum Qualifications
2+ years of related education, experience, or certification.
Current BLS or CPR Certification required.
Internet connectivity with minimum speeds: 3.8 Mbps (down) / 3.0 Mbps (up); latency acceptable as needed.
Efficient and reliable transportation, including an active driver’s license, allowing travel across an assigned region to meet patient needs (locations may include offices, clinics, and patient homes).
Preferred Qualifications
Active Community Health Worker (CHW) Certification.
Customer service experience.
Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook.

Compensation
Hourly Range: $22.25-$25.00
