The Care Coordinator is responsible for receiving requests for service and information; opening CRIA (Communication, Referral, Information and Assistance) encounters and following up appropriately; providing Care Coordination for Elderly Virginians (CCEVP Service Coordination Level Two, as defined by the Virginia Department for Aging and Rehabilitative Services), or, if situational elements that can trigger Options Counseling are present and the individual accepts it, Options Counseling; assessing individuals using Virginia’s Uniform Assessment Instrument (UAI); and documenting everything appropriately in Peerplace, Virginia’s No Wrong Door Tool.

  • Accepts referrals for services from older persons, their families, health care providers and the community.
  • Provides Communication, Referral, Information and Assistance service.
  • Performs Service Coordination Level One service delivery elements of outreach, intake/screening, assessment (using the UAI), service planning, service delivery, monitoring, reassessment and termination.
  • Provides Options Counseling, employing person-centered practices to respect the rights of individuals to control and make choices about their own lives.
  • Enters and/or documents everything in Peerplace, Virginia’s No Wrong Door Tool.
  • Acts as responsible advocate for older persons in Planning District 1.
  • Assists with fundraising.
  • Provides Emergency Services, working closely with the Emergency Services Director.
  • Other duties as assigned by the Director of Care Coordination or the Executive Director.
  • Attends agency staff meetings as well as Care Coordination team meetings as required.
