Overview Riverside Community Care is seeking a passionate Care Coordinator to join our Behavioral Health Community Partners (BHCP). The Care Coordinator provides person-centered and recovery-oriented care management and coordination to eligible MassHealth consumers served in the community. Responsibilities include outreach to engage eligible consumers, helping them identify individualized care goals, linking to appropriate community-based resources, communicating with the multi-disciplinary team, and conducting ongoing need assessments. This is a great opportunity to gain experience with community outreach and coordinating services for individuals with medical and behavioral health needs. This is a full-time role (40 hours) with a hybrid schedule offering some remote work and in-person days, and periodic travel within approximately 30 miles of the identified office.
Schedule: Full Time, 40 hours. Hourly rate: $22.95/hour. The position allows a 4- or 5-day work week depending on the arrangement.
Responsibilities Provide outreach to and engage eligible MassHealth consumers in services.
Assist consumers to identify individualized care goals and connect with appropriate community-based resources.
Communicate with the multi-disciplinary team and complete ongoing needs assessments.
Coordinate care management activities to support clients in achieving and maintaining a healthy quality of life.
Maintain documentation and track outcomes as required.
Qualifications Required Experience: Candidates must have one of the following:
Bachelors degree in a behavioral health or social service field.
LADC II Licensure
Associates degree in a behavioral health or social service field with one year of professional or lived experience.
High School Diploma/GED with two years professional or lived experience.
Experience working with substance use disorders preferred.
Experience with MS Office, electronic health records, and email strongly preferred.
Valid drivers license and regular access to reliable transportation required.
