Care Coordinator

Schedule: Monday - Friday 9:00am - 5:00pm


We are seeking a highly motivated and compassionate Care Coordinator to join our healthcare organization. The Care Coordinator will serve as a vital link between our patients and our healthcare team. The successful candidate will be responsible for guiding patients through care plans, ensuring follow-up, and promoting a healthy lifestyle. Additionally, the Care Coordinator will assist in the referral process and be solely responsible for care team-based utilization management, and patient follow-up for hospital discharges and emergency room visits.


Responsibilities:

  • Act as peer support for enrolled patients which includes advocacy as patients navigate the medical system and relationship building with individuals and their families.
  • Manage inpatient and outpatient utilization and medical care follow-up.
  • Perform population-based management activities.
  • Ensure patients health risk assessment are completed and provided to the various entities.
  • Report any health follow-up. Lenges to the care team including Care Coordinator
  • Maintain and distribute approved disease specific education to identified patient panel and community.
  • Provide reminder phone calls for various scheduled appointments or follow-up after scheduled appointments.
  • Assist patients with obtaining specialty care visits, Primary Care Provider visits, and ancillary services such as labs/diagnostics, including making appointments as needed.
  • Manage the referral process by setting up, completing, and tracking medical referrals

Qualifications:

  • Associates Degree or 3 years or more of health related experience. Bachelor's Degree preferred.
  • Proficient in Electronic Health Record, MHN Connect, Microsoft Word and Excel.
  • A minimum of three (3) or more years of experience or related healthcare experience required.

Requirements:

Education and Experience
