Job Description
The Care Coordinator serves as a point of contact for patients, families, and healthcare providers to ensure seamless, patient-centered care. This role involves communicating with patients regarding next steps in their treatment and care plans, scheduling follow-up appointments, and discussing additional resources available through the clinic and the county.
Responsibilities
Serve as a primary point of contact for patients, families, and healthcare providers.
Communicate with patients regarding their treatment and care plans.
Assist patients in scheduling follow-up appointments.
Discuss and provide information about additional resources available through the clinic and the county.
Ensure that required steps for additional care resources are being taken.
Essential Skills
Strong communication and interpersonal skills.
Excellent organizational and time management abilities.
Proficiency in Microsoft Office and electronic health record systems.
Experience in healthcare settings.
Additional Skills & Qualifications
High school diploma or equivalent.
BLS certification (assistance available for obtaining this certification).
Work Environment
This position is based on-site at a community health clinic, working Monday through Friday during clinic hours. The work environment offers an excellent opportunity to gain experience supporting a Care Coordination team, which can be challenging to acquire for lower-level healthcare employees.
Job Type & Location
This is a Contract position based out of Minneapolis, MN.
Pay And Benefits
The pay range for this position is $21.00 – $31.00 per hour.
