Care Coordinator

Schedule:

Full-time, Monday through Friday from 8:00 am to 5:00 pm

Position Description:

The Care Coordinator assists in providing proactive whole person care management services that addresses the needs of individual patients. They support care managers and work closely with families, providers, clinical teams, and community resources to enhance the health outcomes of our pediatric patients. The Care Coordinator is responsible for outreach and navigation of healthcare services, community-based organizations, schools, state and local-services.

Key Responsibilities:

• Pro-actively outreach to current and prospective patients to educate them on services available to them.
• Serve as a welcoming, empathetic point of contact for patients and families in need of care coordination support.
• Build trusting, long-term relationships with patients that encourages engagement with care management services.
• Provide coaching and motivational interviewing to engage families, support behavior change, and empower participation in achieving health goals.
• Identify and address barriers for care coordination and appropriate care management interventions.
• Connect families with community resources including food assistance, housing support, mental health services, and educational programs.
• Assist in communication and coordinating across health care providers, community-based organizations, schools, and state- and local-services to address patient’s needs and goals.
• Assist with obtaining records from and sending records to outside providers and resources.
• Maintain an up-to-date directory of local community resources and services.
• Identify and promptly escalate clinical, psychosocial, and/or operational issues to care manager, supervisor, or primary care provider to ensure timely resolution.
• Perform outreach and assist in appointment scheduling for patients with outstanding quality gaps.
• Completion of patient agendas as requested by various insurance organizations.
• Communicate recommended activities to primary care providers to enable quality gap closure at point-of-care.
• Support and participate in community events aimed at quality improvement (i.e., vaccine clinic).
• Monitor performance and collect data as needed to support quality reporting.
• Track and document outreach, resource referrals, and patient interactions in care management platform, electronic health record (EHR), and other various systems of record (systems may vary by insurance type).
• Demonstrate cultural competence and sensitivity in working with culturally diverse patient populations.
• Frequently update and work closely with other care team members including providers, clinical teams, care managers, care navigators, referral coordinators, community resources, patients and families, and other care team members.
• Participate in care team huddles, case conferences, quality improvement and program meetings.
• Ability to travel to local practices and community agencies as needed.
• Participation as necessary in the practices on call coverage system to ensure patients are appropriately supported.
• Flexibility of work schedule is required to support the patient.
• Perform other duties as assigned by their supervisor within this scope.

Requirements:

• 2 years of experience providing support to patients in a professional role
• High school diploma or equivalent

Preferred Qualifications:

• Bachelor’s Degree
• Pediatrics experience
• Bilingual Spanish
• Prior experience working in a healthcare setting with strong organizational and computer skills
• Passionate and committed to improving the health and well-being of children and their families
• Solution-oriented and able to navigate challenging people situations
• High emotional intelligence with excellent communication and interpersonal skills, and the ability to build trust with patients and families
• Ability to thrive in a fast-paced, dynamic environment and adapt to changing priorities

• Ability to apply and utilize evidence-based clinical guidelines
• Comfort conducting home visits or providing services in the community
