Care Coordinator

About the Role:

The Care Coordinator plays a pivotal role in the healthcare system by ensuring that patients receive comprehensive and continuous care throughout their treatment journey. Minimum Qualifications:

• MA or LPN
• Experience in a healthcare setting, preferably in care coordination or case management.
• Strong organizational and communication skills.

Responsibilities:
