Position Requirements

Bachelor’s or master’s degree in Psychology, Sociology, Social Work, Nursing, Counseling, or Human Services Administration.

Valid driver’s license and an insured vehicle.

SCI Summary
SCI supports people with disabilities, behavioral challenges, medically complex needs, transitioning youth, the elderly, and other populations using Maryland Department of Health’s Targeted Case Management and Supports Planning work models.

Position Summary
Provide case management services and support to individuals with disabilities and/or medical needs requiring assistance with Activities of Daily Living (bathing, dressing, toileting, eating, mobility). Primary objectives include building positive relationships, exploring options, establishing and monitoring Person‑Centered Plans of Service, securing ongoing support services, and ensuring state‑mandated requirements are met.

SCI Team Member Expectations
All SCI Team Members are expected to follow the organization’s mission and values, HIPAA confidentiality, collaborate with peers, participate in leadership meetings and trainings, comply with employment laws, practice self‑direction and person‑centered planning, and foster a culture of support. Supports Planners provide targeted case management by advocating, arranging, monitoring services, and identifying supports for Medicaid Home and Community Based services.

Essential Duties

Advocate and educate people served and their support system on self‑determination and independence.

Collect eligibility information and documentation for Supports Planning services.

Plan, facilitate, develop, and continuously update Person‑Centered Plans of Service and related tools.

Research, refer, and coordinate resources to meet Person‑Centered Outcomes and share resources with team members.

Monitor Plans, conduct follow‑up activities, perform on‑site visits, complete required paperwork, and manage regulatory compliance.

Adhere to SCI policies and procedures, maintain HIPAA records, use tracking tools, and meet performance benchmarks.

Minimum Qualifications
Education: Bachelor’s or Master’s degree in a human service field (Human Services Administration, Psychology, Sociology, Social Work, Nursing, Counseling).

Experience working with people with disabilities.

Experience with person‑centered planning.

Physical Demands
Standard office environment. Remain stationary >95% of the time. Operate computer, keyboard, mouse, printer, mobile smartphone. Occasionally move 10-25 pounds of office equipment. Good vision for close work. Ability to operate a motor vehicle.
