Objective:

Provide essential case management duties to clients served on the Waiver program. Work with a multidisciplinary team to assist clients receive respectful, responsive, culturally appropriate services that will enable the clients served to lead expressive and determined lives.

JOB SUMMARY: As a Case Manager, you will conduct assessments, create and review care plans, and coordinate services with individuals and multidisciplinary team. Case Manager will also connect individuals served with community resources and ensure waiver coordination and documents meet agency guidelines.

Responsibilities:

· Support clients on the CADI, BI, EW and DD waivers or Alternative Care program in Hennepin County to access needed waiver and state plan services, as well as needed medical, social, education and other services.

· Cultivate and device a plan of care (CSSP) that is based on the MNChoice assessment outcomes and within the county approved allocation for the client.

· Meet with clients in their home, assisted living facility, group homes throughout Hennepin County.

· Manage client intakes, discharges, meetings and services identified in the service plan.

· Monitor services provided to the client to evaluate the effectiveness and assure safety.

· Safeguard client right to privacy, compliance with HIPAA and all county and state regulations.

· Monitor waiver budgets and waiver allocations to confirm appropriate service consumption. Report spending issues promptly to Case Management Program Manager.

· Maintain billable time and accurately document services delivered.

· Manage a yearly comprehensive assessment to determine healthcare, social, housing, educational, vocational and other service needs.

· Ascertain service needs and make referrals based on assessment, current client need and individualized plan.

· Monitor statewide or county changes in regulations in waiver case management funding and modify services as needed.

· Other duties as assigned.

Competencies:

· Culturally competent.

· Strong organizational and prioritizing skills.

· Excellent written and verbal communication skills.

· Able to work individually and collaborate with multidisciplinary members.

· Completion of Person Centered Training (not required, but preferred).

· Climb flights of stairs.

· Lift 40 pounds or more.

CEU Requirements:

· 30 Continuing Education (CEU) Credits every 2 years.
