Overview This position provides monitoring, linking and coordination of services to individuals with a mental health diagnosis, in Elk and Cameron Counties. Provides wellness coaching to designated consumers along with coordinating these activities with the Wellness Nurse. Ensures adherence to state and provider regulatory standards. This position also participates in an on-call schedule. This position may also provide Administrative Case Management services to individuals on their caseload as those consumers no longer require the intensity of the Blended Case Management program.

POSITION QUALIFICATIONS Education / Experience
Bachelor's degree with major course work in human service, education or social science field; or
Registered Nurse; or
A high school diploma and 12 semester credit hours in human service, education or social science AND two years of experience in direct contact with mental health consumers.

Valid driver's license.
