ABOUT THE ROLE The Registered Nurse Case Manager (RN Case Manager) will provide direct patient care services to hospice patients and supportive care to the patients family; manage an interdisciplinary group (IDG) consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care; and case manage all assigned patient clinical records.
Schedule: MondayFriday 8:00?am5:00?pm.
ROLE REQUIREMENTS
Registered Nurse of an accredited school of nursing with a current license to practice professional nursing in the state in which we do business.
One (1) year of experience in oncology, long?term care, hospice or home health setting.
Current Cardiopulmonary resuscitation (CPR) certification.
Must have reliable transportation, proof of current automobile insurance and a valid drivers license.
Candidate must be willing to travel.
Candidate must have 1 year of hospice experience.
