About the role: The Bilingual Registered Nurse Case Manager plans, organizes, and directs hospice care. The professional nurse builds from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities. Bilingual (English/Spanish) required.
Job Responsibilities
Patient Care Completes an initial, comprehensive assessment of the patient and family to determine hospice needs; provides a complete physical assessment and history of current and previous illness(es).
Provides professional nursing care by utilizing all elements of the nursing process.
Assesses and evaluates patient status by writing and initiating a plan of care; regularly re-evaluates patient and family/caregiver needs; revises the plan of care as necessary; initiates revisions as patient status changes.
Uses health assessment data to determine nursing diagnosis and develops a care plan with patient and family involvement; initiates preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed.
Counsels the patient and family in meeting nursing and related needs; provides health care instructions as appropriate; assists with activities of daily living and promotes self-sufficiency and comfort care.
Acts as Case Manager when assigned and coordinates patient care for the caseload.

Communication Completes and maintains accurate clinical notes; communicates with the physician regarding patient needs and changes; coordinates with community health-related persons to align the care plan; provides education and counseling to patients and families; ensures a safe environment; works with the interdisciplinary group.
Attends interdisciplinary group meetings and supports continuity of care.

Additional Duties Participates in on-call duties; ensures equipment and services are available; supervises ancillary personnel as required; pursues ongoing professional development; participates in quality assessment and performance improvement activities.

Job Qualifications Graduate of an accredited school of nursing. Preferred: 12 years of recent acute care experience in an institutional setting, and 12 years in home care.
Current licensure in state and CPR certification.
Bachelors degree, with hospice care experience preferred.
Management experience is not required; may supervise hospice aides.
Excellent observation, communication, problem-solving, and basic math skills; nursing skills per competency checklist.
Physical ability to perform duties (standing/walking; lift/push/pull as needed; ability to lift up to 50 pounds); valid drivers license with insured vehicle.
Bilingual (English/Spanish) required: comfortable speaking both languages.
