Job Responsibilities Completes an initial, comprehensive, and ongoing assessment of the patient and family to determine hospice needs.
Provides professional nursing care using all elements of the nursing process.
Writes and initiates a plan of care, regularly re‑evaluates patient and family/caregiver needs, and revises the plan as necessary.
Uses health assessment data to determine nursing diagnosis and develops a care plan that establishes goals based on this diagnosis.
Initiates appropriate preventive and rehabilitative nursing procedures, administers medications and treatments as prescribed by the physician, counsels the patient and family, and provides health care instructions.
Assists the patient with activities of daily living and facilitates self‑sufficiency and optional comfort care.
Acts as Case Manager when assigned by Clinical Supervisor and coordinates patient care for the assigned caseload.
Communication Maintains accurate and relevant clinical notes, records pain/symptom management changes/outcomes, and communicates with the physician regarding patient needs.
Coordinates care with community health‑related persons and teaches self‑care techniques to patients and families.
Works in concert with the interdisciplinary group, provides and maintains a safe environment, and attends interdisciplinary group meetings.
Additional Duties Participates in on‑call duties as defined by policy.
Ensures arrangements for equipment and other necessary items and services are available.
Supervises ancillary personnel and delegates responsibilities when required.
Maintains professional knowledge through continuing education and in‑service classes.
Participates in quality assessment performance improvement teams and activities.
Job Qualifications Graduate of an accredited school of nursing; one to two years of recent acute care and home‑care experience.
Current licensure in nursing and CPR certification; bachelor’s degree preferred.
Must be bilingual (English/Spanish) and can interact with clients in both languages.
Excellent observation, communication, problem‑solving, math skills, and competency checklist completion.
Physical ability to lift, move, and transfer patients; lift up to 50 pounds.
Licensed driver with an insured automobile in good working order.

The employee must frequently sit or stand and occasionally lift and/or move up to 50 pounds while performing the duties of this job.
