We are thrilled to announce an exciting opportunity to join our compassionate team at Capital Hospice & Palliative Care! We are currently seeking a full-time hospice RN who is passionate about providing exceptional care to patients and their families.
As a hospice RN, you will play a vital role in organizing and directing hospice care, utilizing your nursing expertise with a focus on community health education. Your responsibilities will include assessing patient needs, planning and delivering personalized care, and collaborating effectively with patients, families, and our interdisciplinary team.

Reports To: Clinical Director
Patient Care Responsibilities
Completes an initial comprehensive physical assessment and history of current and previous illnesses.
Ongoing comprehensive assessment of patient and family to determine hospice plan of care.
Develop and revise care plans in collaboration with IDG, PCP, patient & families.
Initially and continually educates patient and families on hospice services & philosophy.
Establish HHA care plans as well as indirectly and directly supervising the plan of care per regulations.
Excellent communication with Clinical Director, IDG Team, PCP, community health representatives, patient & family.
Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
Knowledge of and availability to perform patient information visits or admissions as needed including explanation of the hospice services.
Participate in on-call rotation as prescribed by the needs of the agency to provide nursing service to patients when required outside office hours.
Attend and participate in interdisciplinary group meetings.
Qualifications
Graduate of an accredited school of nursing
Valid license as a Registered Nurse in the State of Illinois
Valid IL driver's license and auto insurance
