Summary
The Bereavement Counselor LCSW will provide bereavement support to patients, families, and hospital staff as part of the Palliative Care Interdisciplinary Team.
Responsibilities
Conduct in-person and follow-up counseling sessions for individuals coping with grief and loss. Provide clinical assessment, counseling, and support to the bereaved.
Provide bereavement support to patients, families, and hospital staff.
Conduct in-person and follow-up counseling sessions.
Develop and provide bereavement services, including support groups and individual support strategies.
Facilitate linkage to community services for bereavement.
Participate in the orientation, education, and training of new staff.
Qualifications
Master’s Degree in Social Work required. Current State of Illinois licensure as a Licensed Clinical Social Worker (LCSW) required. Two years of previous social work experience in healthcare, care management, mental health, and/or social services preferred.

Strong written and verbal communication skills.
Proficiency with the Microsoft Office Suite.
