Behavioral Health Specialist Case Manager; Permanent Supportive Housing
We’re seeking two (2) full‑time Behavioral Health Specialist Case Managers for Transitional Age Youth (TAY) at the Lotus to provide comprehensive case management and behavioral health support services to youth aged 18‑24 who are experiencing housing instability or homelessness. The Case Manager works collaboratively with strategic partners, using a trauma‑informed approach to support youth in maintaining housing. This role is critical in ensuring housing retention by delivering high‑quality, individualized services that address both housing needs and behavioral health challenges.

The Professional I Behavioral Health Specialist, Case Manager contributes to the PSH Care Team by providing strength‑based case management services to individuals with chronic homelessness and behavioral health conditions. The role includes integrated care coordination, ensuring whole‑person health needs are met, including medical, behavioral health, and substance misuse services, and facilitating linkages to community resources.

Ideal Candidate Will Have

High school diploma with at least 3 years of case management or care coordination experience; a bachelor’s degree preferred.

Prior experience working with serious and persistent mental illness (SPMI) youth.

Working knowledge of developmental stages, growth, and family dynamics.

Valid driver’s license in the State of Ohio, reliable vehicle for client transportation, and willingness to collaborate closely with team members.

Ability to form effective therapeutic relationships, engage clients in mutual assessment of problems, goal setting, and coordinate care plans from initial contact through end of service.
