Travel and Schedule
This position requires some field-based travel (up to 15%) in the Twin Cities area meeting with members in facilities, the community or in their home. You will work from home (telecommuting) performing telephonic case management when not in the field. The hours for this position are Monday through Friday, 8:00 am – 5:00 pm CST.
Primary Responsibilities
Provide telephonic care coordination to MN Medicaid and Medicare members with a mental health disability.
Complete annual face-to-face meetings for qualified members.
Manage gaps in care for members in both their mental health and medical needs (e.g., readmissions to higher levels of care, annual exams/services).
Manage member transitions to and from acute care settings (admissions/discharges).
Coordinate and collaborate care with providers, facilities, and supports.

Required Qualifications
Master's Degree in Psychology, Social Work, Counseling, or related field; or Bachelor's Degree or greater in Nursing
Active, unrestricted, independent (fully licensed) license in Minnesota: LP, LPCC, LICSW, LMFT or RN
2+ years of clinical experience in a behavioral health setting
Live in or near the Twin Cities, MN
Ability to travel within the metro for annual member visits (up to 15%)
