Role Overview
The Behavioral Health Case Manager II performs case management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse needs, and is a subject matter expert in targeted clinical areas such as Eating Disorders, Maternity Alcohol/Drug, Autism Spectrum Disorders, etc.
How You Will Make An Impact
Respond to complex cases and account‑specific requests.
Use appropriate screening criteria knowledge and clinical judgment to assess member needs.
Conduct assessments to identify individual needs and develop specific care plans.
Monitor and evaluate effectiveness of care plans and modify as needed.
Support member access to quality, cost‑effective care and coordinate with internal and external resources.
Serve as a resource to other BH Case Managers.
Participate in cross‑functional team projects and initiatives.
Minimum Requirements
MA/MS in social work counseling or a related behavioral health field, or a degree in nursing; minimum 3 years of clinical experience in social work counseling with complex psychiatric and substance abuse treatment.
Current active unrestricted license (RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, Clinical Psychologist, etc.) as allowed by applicable state laws.
Previous experience in case management and telephonic/in‑person coaching with members with complex psychiatric/substance abuse disorders; managed care experience required.
For Puerto Rico candidates: current PR health certificate and PR Law 300 certificate.
Government or 311 licenses acceptable per state law.
Preferred Skills, Capabilities and Experiences
Strong knowledge of social and community resources within the greater Massachusetts area.
Experience in health coaching and motivational interviewing techniques (preferred).
Salary and Benefits
The salary range for this position is $68,400 to $117,990. Elevance Health offers a comprehensive benefits package including 401(k) match, health, dental, vision, disability, and equity stock purchase options.
