Overview
The Behavioral Health Case Manager (BHCM) supports individuals with severe mental illness (SMI), serious emotional disturbance (SED), and/or co-occurring substance use disorders. Many clients are unhoused, recently discharged from inpatient psychiatric care, or frequently use emergency or crisis services. Face-to-face, field-based engagement is essential.
Key Responsibilitiesh3>
Field-Based Client Engagement & Intensive Support (30–35%) : Conduct frequent in-person visits in the community (homes, shelters, clinics, encampments, or hospitals); build and sustain trusting, therapeutic relationships; use trauma-informed, harm reduction, and motivational interviewing techniques; provide psychoeducation, emotional support, and advocacy tailored to behavioral health conditions.
Care Coordination & Case Management (25–30%) : Lead development and updates of individualized, strengths-based care plans; coordinate with psychiatrists, therapists, primary care, crisis teams, housing case managers, and justice system partners; support hospital and psychiatric discharge planning.
Behavioral Health Assessment & Monitoring (15–20%) : Conduct initial and ongoing behavioral health assessments; identify and address barriers to medication adherence; monitor members in real time through home visits, phone touch-bases, community observation, and collateral contacts.
Documentation & Compliance Reporting (10–15%) : Document all encounters and activities per ECM and Medi-Cal standards; track clinical outcomes, engagement attempts, and service delivery to support quality improvement and funding requirements.
Collaboration with Behavioral Health & Cross-Sector Teams (5–10%) : Participate in multidisciplinary team meetings; share observations from in-person visits to inform clinical decision-making and crisis planning.
Qualifications
At least one of the following master’s degree is required: ASW, LCSW, LMFT, AMFT, LPCC, MSW, APCC
Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, Teams, etc.)
Strong organizational, time management, and multitasking skills
Excellent written and verbal communication skills
Ability to work independently and as part of a team
High level of initiative and problem-solving ability
Onsite position in Redlands, California – Standard office hours: 8:30 AM – 5:30 PM
