Essential Functions

Provide skilled nursing services during 16‑hour weekend shifts (paid for 24 hours) across Boston, Braintree, Quincy, Everett, and Revere.

Complete intake, annual assessment, and admission visits.

Maintain all required documentation, including client assessment forms and multidisciplinary plan of care, and communicate professionally with the client’s PCP and other healthcare providers.

Review the client admission binder, educate caregivers on the caregiver communication tool or electronic visit verification system.

Perform regular in‑home visits, caregiver education/skills training, and assess health and environmental changes.

Conduct semi‑annual visits to review and update care plans, medication reconciliation, and caregiver communication tools.

Address concerns related to environment, psychosocial status, and physical health changes.

Participate in multidisciplinary team meetings and collaborate to address client needs.

Foster a culture of customer service and commitment to quality care.

Serve as a brand ambassador for Vitra Health, reflecting our vision, mission, and values.

Show genuine interest, compassion, and commitment to the diversity of our clients and team members.

Other duties as assigned.

Physical and Environmental Demands

Work in a clean, well‑lit environment with fluctuating temperatures, often in close proximity to others.

Perform substantial periods of repetitive work using computer equipment.

Lift and carry equipment and supplies up to 35 pounds; push/pull equipment up to 35 pounds; walk, stand, sit frequently; navigate stairs; maintain visual acuity and manual dexterity.
