The Hospital of the University of Pennsylvania is seeking an experienced Advanced Practice Provider (APP) to join the inpatient Palliative Care Program as a Clinical Coordinator. Palliative Care specialists provide pain and symptom management, psychosocial support, and advance care planning for patients and families facing serious illness. APPs on the Palliative Care team are responsible for working as consultants throughout the hospital, visiting directly with patients and families and offering recommendations to the primary healthcare team.

The APP Clinical Coordinator will provide direct supervision of APPs on the Palliative Care team, including scheduling, payroll management, and performance review, as well as serving as a clinical resource for direct reports. The Clinical Coordinator will partner with members of the interdisciplinary Palliative Care leadership team to support and optimize daily operations, productivity, quality initiatives, and team wellness. In addition to administrative duties, the Clinical Coordinator will serve as a core member of the Palliative Care consultant team. This includes providing expert symptom management and guiding crucial discussions with patients and families to promote a personalized, comprehensive care plan that aligns with patient goals. The Clinical Coordinator will participate in daily inter-professional palliative care team rounds to support the interdisciplinary practice model and to facilitate team communication and collaboration.

Responsibilities include:

  • Providing direct clinical care for patient population according to established protocols and utilizing evidence based practice standards in collaboration with team.
  • Maintaining accurate and confidential medical records.
  • Performing interventions specific to assigned patient population and collaborative agreement.
  • Participating in the performance improvement processes to meet quality of care and service standards.
  • Demonstrating awareness of professional responsibility as role model to peers and members of the health care team.
  • Performing approved therapeutic or diagnostic procedures based upon patient's clinical status.
  • Addressing patient and family concerns and creating a therapeutic relationship to provide support.
  • Providing patient education, counseling and instruction regarding patient diagnosis, medical treatments, medications, and preventative care.
  • Participating in and supporting accreditation, compliance and regulatory activities of the organization.
  • Precepting NP students and other health care learners.
  • Identifying patient/family needs for community resources and referring appropriately.

Minimum Requirements:

  • Master's prepared Nurse Practitioner with Acute Care certification.
  • Current Pennsylvania license (or eligible to obtain).
  • A minimum of 2-3 years of experience as an Advanced Practice Provider in the acute care setting.
  • Training/experience in palliative care, hospice, or geriatrics is strongly preferred.
  • Prior experience in a supervisory or leadership role is strongly preferred.
  • Candidates should possess strong communication skills and the ability to work independently with a wide variety of medical and surgical services.

Credentials:

  • ACLS (For providers in the acute care setting) required.
  • BLS/CPR, as a healthcare provider as per the American Heart Association (For providers in the ambulatory setting) required.
  • PA and/or NJ CRNP licensure (CRNP) required.
  • PA and/or NJ Prescriptive Authority required.
  • PA and/or NJ RN licensure (RN) required. National Board Certification.

Education or Equivalent Experience:

  • Master of Arts or Science (Required)
  • Education Specialization: Master's Degree or Doctor of Nursing Practice (DNP) in Nursing
  • Equivalent Experience: And New to practice or < 2.5 years, The CRNP must be reviewed by the Credentials Committee and approved by the Medical Board.
