About the Role
The Assistant Geriatric Care Manager supports aging and older clients by coordinating, monitoring, and delivering culturally sensitive, trauma-informed care. Working closely with our Adult Services team, you’ll help clients maintain independence, dignity, and quality of life while connecting them to critical community resources.
Key Responsibilities
Support Geriatric Care Managers in implementing individualized care plans.
Coordinate services such as home care, medical appointments, housing, and benefits.
Serve as a liaison between clients, families, and providers.
Maintain documentation and case records in compliance with agency standards.
Assist with events, outreach, and administrative support for the Adult Services team.
What We’re Looking For
Bachelor’s degree in social work, gerontology, psychology, nursing, or related field (or equivalent experience).
1–2 years of experience working with older adults or in social services.
Knowledge of community resources and public benefit programs.
Excellent interpersonal, organizational, and communication skills.
Reliable transportation and proficiency in Microsoft Office and case management systems.
