They serve as a role model, actively listening, observing, and respecting the preferences and interests of participants while complying with all applicable human rights and funding regulations.

Title: Assistant Case Manager (815)

Department: Day Habilitation

Essential Duties and Responsibilities

Provide individual and small group instruction in self‑care, activities of daily living, communication, social, and motor skills.

Provide direct care for ADLs and group activities; lift and transfer individuals as appropriate.

Participate in scheduled supervision sessions with Assistant Director.

Communicate respectfully and proactively with families, residential staff, and all team members.

Attend program‑wide staff meetings and case review meetings as scheduled.

Participate in team meetings regarding general program development, curriculum development, and schedules.

Attend in‑service trainings as required.

Comply with all applicable state and federal regulations, agency policy, and personnel practices.

Protect the health and safety of all participants and staff.

Demonstrate excellent interpersonal skills.

Work effectively in a team environment.

Perform other duties as requested by supervisor(s).

Supervisory Responsibilities: This job has no supervisory responsibilities.

Qualifications
Education and Experience: High school diploma or equivalent. Direct experience working with individuals diagnosed with intellectual and developmental disabilities and their families preferred.

Language Skills: Ability to read, interpret, write routine reports, and speak effectively before groups.

Mathematical Skills: Ability to add, subtract, multiply, divide, compute rates, ratios, and percentages, and interpret bar graphs.

Reasoning Ability: Apply common‑sense understanding to detailed written or oral instructions and solve problems involving concrete variables.

Vehicle and Travel: Requires a legally insured and inspected automobile for work use and availability during working hours.

Certificates, Licenses, and Registrations: Valid driver’s license, CPR, and First Aid certification.

Physical Demands: Regular standing, walking, sitting, using hands, reaching, talking, hearing, climbing, balancing, stooping, kneeling, crouching, crawling. Occasionally lift up to 50 lbs. Vision: peripheral, depth perception, focus adjustment.

Work Environment: Exposure to outside weather, wet or humid conditions, moving mechanical parts, vibration, moderate noise levels.
