Required Education: MSN for Nurse Practitioners or BS for Physician Assistants

Required Related Experience: 1 yr

Required License/Registration/Certification: Advanced Practice Registered Nurse (APRN) with Neonatal Nurse Practitioner (NNP) certification or Physician Assistant-Certified (PA-C). Basic Life Support (BLS), Pediatric Advance Life Support (PALS), and Neonatal Resuscitation Program (NRP) within 90 days of hire.

Physical Requirements: Lifting, pushing, or pulling of objects up to 50 lbs, sitting for long periods of time.

Shift: Monday- through Friday, equivalent to two 12-hour shifts in the NICU and two 8-hour shifts in the clinic setting.

Job Summary: The Advanced Practice Provider (APP) functions within the pediatric chronic care team ambulatory setting and within the neonatal ICU to manage the needs of the chronically ill patients. The APP with care for chronically ill patients within the in-patient NICU setting to help prepare the patient and guardians for transition to discharge. This includes but not limited to assisting in preparation for discharge, including coordination of home medical devices such as oxygen, feeding tubes, and monitors. Post-discharge, the APP will provide follow-up care in the ambulatory setting including telehealth, collaborating with outpatient services and case management to ensure continuity of care and support for families transitioning to home. The APP with care for the patient in an ambulatory setting while working closely with outpatient testing groups and the case management team. The APP is on a service aligned team and manages the care of the outpatient with the team and day to day patient care from ICU to step down to discharge. The APP will have responsibilities for this same patient population in the consultative inpatient setting or serves a resource for the inpatient care team and for follow up in the outpatient setting. The APP must have a strong foundation in evidenced based practice, collaboration of care and education. The APP will function within the NICU and throughout the KCH service line and is an integral member of a multidisciplinary health care team.
