Hospice Registered Nurse Case Manager / Alt. Director of Clinical Services
to help cover our newest territory in Dallas (Carrollton & Surrounding Area)
Constellation Health Services is seeking a compassionate and skilled Alt. Director of Clinical Services / Registered Nurse Case Manager to work with hospice patients and their families in their homes, assisted living facilities, and skilled nursing facilities.

Responsibilities as a Registered Nurse Case Manager: Provide skilled nursing care, including medication administration and treatments as prescribed in the physician's plan of care for patients in their homes.
Perform comprehensive assessments and coordinate care plans with all involved disciplines, including family and primary caregivers.
Observe, evaluate, and report on patients' physical and emotional status to the nursing supervisor and physician when changes occur.
Participate in weekly Interdisciplinary Team Meetings, providing updates on patients' physical, emotional, psychosocial, and spiritual well-being.
Maintain and process documentation in compliance with agency standards, utilizing a computer-based system.
Submit all required clinical documentation, time records, and other information within designated timeframes.
Support the Home Health Aide Supervisor as needed for hospice aide evaluations.
Provide weekend and holiday coverage as needed.
Responsibilities as an Alternate Director of Clinical Services: Oversee all patient services provided by clinical staff.
Guide and counsel clinical staff to improve hospice care services.
Assist in managing clinical teams and planning.
Contribute to assessment, planning, implementation, and evaluation of patient and family care.
Assist in budget preparation and administration.
Monitor operational indicators to adjust staffing and resources accordingly.
Ensure clinical records comply with legal standards.
Participate in performance evaluation and quality assurance programs.
Develop organizational goals and policies.
Ensure compliance with licensure and certification standards, including CHAP standards.

Qualifications Licensed Registered Nurse (RN) with the required licensure, education, and qualifications per state law.
Graduate of an NLN-accredited nursing program with a BSN, or an associate degree/diploma with at least three years of recent clinical experience, including at least one year in home health or community health care.
Certification by the American Nurses Association as a community health nurse or coursework in healthcare management.
Experience in hospice or home care, with supervisory or management experience preferred.
Adherence to professional standards and excellent communication skills.
Understanding of hospice philosophy, pain and symptom management, and issues related to death and dying.
Valid driver’s license, insured vehicle, and good driving record.
