MUST HAVE ACTIVE ALASKA LICENSE MUST HAVE FLU VACCINATION ACCOUNTABILITIES Responsible for the active management of SEARHC patients. a. Collaborates with the medical provider and allinterdisciplinary team members to establish individualized medical goals thatmaximize patient healthcare outcomes. b. Uses problem-solving, critical thinking and anticipatoryskills in meeting the needs of the patient and the provider. Patient needs should be addressed in aprompt, efficient manner. 2. Works closely with providers and other clinical staff to facilitate andcoordinate patient referrals to Surgery/Specialty Clinic. Reviews referral records to ensure allappropriate information is available for providers'review. Tracks the progress of referralsand outcomes of care, providing periodic status report to providers onoutstanding referrals. 3. Serves as the primary clinical contact regarding patient care andin care coordination for the patients. Proactivelyidentifies delays and obstacles to care and works to remedy individual casesand improve the process. 4. Performs staff nursing duties in the various nursing rolesin the clinic. Duties includeassessment, documentation, education and administering appropriate medicationsand/or therapies. 5. Maintains annual competencies including safety and infectioncontrol requirements. Performs duties asassigned in a responsible, respectful and caring manner to promote effectivenursing staff relationships. Maintains aclean and safe working environment including adequate supplies and upkeep ofunit and equipment, reporting any defective or missing equipment and safetyhazards. Adheres to infection controlpolicy and procedures. 6. Communications and Teamwork a. Works collaboratively by building bridges and creatingrapport with team members within departments and across the organization. b. Participates in department and clinic team and improvementefforts. 7. Performs other duties as assigned, including specialprojects deemed necessary to provide comprehensive health services. POSITION QUALIFICATIONS Education: Graduateof an accredited school of nursing (Bachelor's degree preferred) Highschool diploma or GED Experience: Atleast 2 years of general nursing experience Licensure& Certification: A valid, current, full, and unrestricted RN license in the State of Alaska Basic Life Support certification Knowledge,Skills & Abilities: Knowledgeof: Thenursing process and the ability to apply this knowledge in the working environment Customerservice principles Safetyand infection control principles Skillsin: Assessment,anticipation of needs, and data collection Oraland written communications Abilityto: Multi-task and work independently in fast paced environment Team-player Willing to teach when appropriate Problem-solve and use critical thinking skills to develop and maintain good working relations with other staff and departments Willingness to ask for help when encountering a new or challenging situation Perform job using theory-based nursing practice model, evidence based practices andshared governance in a multi-disciplinary team.
