Franciscan Physician Network is seeking a full-time Nurse Practitioner or Physician Assistant with Palliative Care experience for a position at Franciscan Hospice & Home Care in Lafayette, IN to meet the growing demands for outpatient Hospice and Palliative Care services. Provider will see patients in various venues of care such as personal residence, assisted living facilities, and skilled nursing facilities.
Ideal candidate will have 2 to 5 years of Palliative, Home Health or Primary Care experience, with a focus on clinical excellence and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care.
Schedule hours: Monday Friday, 8:00 a.m. 5:00 p.m.; No weekends or holidays
Home based and community facility visits for pain and symptom management
Coverage area from Lake County to Vermillion County; Focus on northwest IN counties is available
Offices located in Lafayette and Hobart, IN
Average patient volume: 5 to 10 patients per day
Experience in Palliative Care, Home Health Care, or Primary Care preferred
Telehealth offered
Master's Degree - Required
Physician Assistant Certified (PA-C) Required - or
Nurse Practitioner (NP) or Nationally Recognized Certification - Required
Basic Life Support Program (BLS) - American Heart Association Required
Travel is required: Up to 75%
