Job Summary We are seeking a part time Adult Case Manager to join our team. In this role, you will work collaboratively with adults with intellectual/developmental disabilities and their families to determine their needs, develop the best course of action, and monitor progress to ensure the delivery of high-quality, individualized support. The ideal candidate is compassionate, patient, and knowledgeable about behavioral healthcare and developmental disabilities.
Responsibilities
Conduct in person, virtual and phone contacts for referrals, meetings and troubleshooting
Collaborate with individuals, families and mentors and facilitate communications
Coordinate scheduling
Develop individualized care plans
Work with mentors to monitor progress toward goals
15 to 20 hours per week - primarily mid-afternoon/early evening hours
Qualifications
Previous experience as a Case Manager or similar position preferred
Experience with intellectual/developmental disabilities preferred
Strong problem-solving and organizational skills
Ability to manage multiple projects or tasks and prioritize appropriately
Ability to work in fast-paced situations and make sound decisions quickly
Excellent interpersonal skills and high level of compassion
Strong verbal and written communication skills
