Position: AB109 Case Manager

Status: Non-Exempt

Report to: Program Supervisor

Hours: 40 hours per week, 8:00 a.m. – 5:00 p.m. (flexible hours depending on client needs; may require evening and/or weekend work).

DESCRIPTION
The AB109 Case Manager (CM) coordinates programming, services, and supports for ex‑offenders during transition or after release from custody in the assigned geographic service area, aiming to promote successful community re‑entry and reduce recidivism.

Responsibilities

Initiate contact, establish rapport, and communicate effectively with ex‑offenders referred to community case management by the correctional system.

Develop a Community Re‑entry Plan in collaboration with the ex‑offender, family, and partner organizations.

Follow up on referrals shared by the correctional system and engage clients in community and family‑oriented activities.

Build a team of informal and formal service providers to support ex‑offenders in implementing their Re‑entry Plan.

Work with the Activities Coordinator/Parent Leader to organize community supports and activities promoting positive peer relations and protective factors.

Engage ex‑offenders in activities at Family Resource Centers or other venues.

Ensure ex‑offender/family support through informal service providers.

Schedule reminders to encourage appointment and activity follow‑through.

Conduct home visits and/or meet participants at diverse and remote sites, including recovery environments and high‑risk neighborhoods.

Maintain accurate case files timely and in accordance with professional standards and funder timelines.

Participate in data collection and evaluation processes.

Enter data into the database and verify accuracy.

Complete daily case notes and ensure database mirroring.

Participate in service integration training, including cross‑training.

Perform other duties necessary to ensure successful integration of services and family strengthening efforts.

Required Knowledge, Skills, and Abilities

Deep understanding of the challenges ex‑offenders face post‑release (psychological, logistical, financial, legal, stigmatization, family, peer).

Ability to work with ex‑offenders and families from diverse ethnic backgrounds.

Reliability and responsibility by attending all scheduled appointments.

Capability to conduct home visits and meet participants in risky environments.

Spatial awareness to avoid dangers when entering high‑risk areas.

Familiarity with the Family Resource Center (FRC) approach to community supports.

Incorporation of family support and protective factor principles.

Broad knowledge of community resources within the local area.

Involvement with grassroots, resident, and neighborhood organizations.

Collaboration with probation officers and other service providers, developing multi‑agency case plans.

Management of mentors working with ex‑offender clients.

Strong personal organization and time management skills; works well under pressure.

Positive framing of client strengths and possibilities.

Facilitation of multi‑dimensional team meetings.

Cooperative system relations while advocating for constructive change.

Basic proficiency in word processing (e.g., MS Word).

Education/Experience

Bachelor’s Degree in criminal justice, social work, or related field and minimum three years working in a community‑based or nonprofit organization; OR 5+ years experience with ex‑offenders or related populations (e.g., gang‑affiliated).

Reliable transportation; up‑to‑date driver’s license and insurance.

Availability for occasional evenings, weekends, and travel.

Flexibility, good sense of humor, and teamwork.

Physical Requirements

Sit for extended periods; stand and walk for long periods, bend over, reach overhead, grasp, push, pull, lift/carry up to 30 lbs.

Operate computer terminal and standard office equipment.

Read printed matter with or without vision aids.

Hear and understand at normal speed, including on the telephone.

Work indoors in a standard office environment and outward outreach to neighborhoods and schools.

Desirable
Bilingual in one of the following languages: Spanish, Pashto, Urdu, Hmong, Vietnamese, Cambodian, or Laotian.

Compensation
Starting pay: $20.06 per hour, based on experience. Full‑time, non‑exempt position subject to overtime and comprehensive benefits (medical, dental, vision, life, retirement). Monday through Friday, 8:00 a.m. – 5:00 p.m.
