ACRS seeks a full-time Punjabi speaking Case Manager I in our Aging and Adult Services department to provide culturally competent case management services to elderly persons and adults living with disabilities within the Asian and Native Hawaiian/Pacific Islander communities. Services are holistic, person-centered, and strengths-based, with the goal of helping individuals live a meaningful and independent life in the community. Duties are performed in accordance with agency policies and contract and regulatory standards, aligning with our core values of Equity, Inclusion, Diversity, and Belonging.

Essential Job Functions Works with clients to develop and implement a person-centered, holistic, and community-based care plan. Tasks include conducting in-home functional assessments and re-assessments addressing life domains, verification of diagnosis, skin condition, medications, treatments, psychosocial information with health care providers as needed; develops, implements, monitors, and revises a written, personalized care plan in collaboration with client/family, health care providers, and other community resources.
Identifies client needs and facilitates access to community resources; manages and follows up on referrals to ensure interventions and services meet the client’s needs.
Authorizes services and client’s choice of qualified provider based on the care plan. Travels to and from home visits and maintains contact to ensure proper implementation of the care plan.
Authorizes or re-authorizes payment for client-approved Medicaid funded services or terminated services from WA State Social Services in Provider One. Informs the client of service changes or termination based on current needs and function.
Provides client advocacy in obtaining services (citizenship, housing, food, medical resources, legal advocacy, translation/interpretation, etc.); makes referrals for LTC resources and assists with financial eligibility review and documentation.
Facilitates client transitions to care settings (Nursing Homes, Assisted Living, Adult Family Homes) through assessment and planning; coordinates with care agencies for placement and continuity of care.
Acts as a mandatory Adult Protective Services (APS) reporter; completes timely reporting of safety concerns and mental/behavioral issues to appropriate responders.
Assists clients and families with home modifications, provides short-term crisis intervention, and helps determine appropriate care settings and hand-offs of care.
Completes documentation within established timeframes; participates in QA, contract compliance, and program improvement initiatives; expected to meet performance and productivity standards.
Attends agency, program, and team meetings, consultations, and trainings.

Position Qualifications Education and Experience: Master’s degree (social work, behavioral or health sciences preferred) with bilingual/bicultural skills relevant to the position, OR Bachelor’s degree with bilingual/bicultural skills, OR Associate of Arts/Science with one year of paid on-the-job social services experience and bilingual/bicultural skills, OR High School Diploma with three years of paid on-the-job experience and bilingual/bicultural skills. Completion of Case Manager training program including State CORE training is beneficial.
Cultural Humility: Ability to communicate effectively with diverse backgrounds; provide respectful, quality care responsive to cultural beliefs and practices; fluency in Asian and Native Hawaiian/Pacific Islander languages preferred; multilingual/multicultural communication valued.
Knowledge and Skills: Customer service with empathy; time management and problem-solving; conflict resolution; knowledge of aging processes, service delivery systems, and related programs; familiarity with illnesses and impact on daily tasks; ability to make sound judgments for safe environments during assessments.
Technical Skills: Proficiency with Microsoft Office and related software; internet research ability.
Additional Requirements: Reliable transportation; valid driver’s license and auto insurance if using a personal or agency vehicle; compliance with all agency policies; ability to work in a hybrid environment.

Working Conditions The position requires mobility within the office and in the field (home visits); may involve sitting for extended periods and lifting up to 20 pounds. The role is hybrid: approximately 60% on-site (home visits) and 40% remote; schedule and location may change at the agency’s discretion. Moderate noise level.
