Key Duties Conduct Palliative Care Visits
Comprehensive assessment, pain and symptom management, patient and family education
Focus on goals of care
Collaborate with interprofessional palliative care team
Support after‑hours, holiday and weekend on‑call
Support provider relationships with clients
Actively support Homebase Medical’s Vision and Goals
Qualifications Active Nurse Practitioner or Physician Assistant license in the State of Washington
Board certification (Nurse Practitioner or Physician Assistant)
DEA licensure
1 year or more of palliative care experience required
CAPC preferred but not required
Experience diagnosing and caring for dementia/Alzheimer’s patients
Compensation $5,000 Sign on Bonus
Stable income + unlimited earning potential
Typical annual earnings: $132,000 – $212,000+
Guaranteed annual base salary: $68,640
Per‑patient rate: $90 for every patient seen
Earning security: each pay period, you’re paid whichever is higher—your base salary or your per‑patient total
$500/month no‑show protection
Mileage reimbursement per IRS guidelines
