Overview 4W/PACU/Yaw 6 ICU Nurse Case Manager, 40 Hours (Days / 4x10 Hour Shifts / Every 6th Weekend)
Position summary: The care manager will assess, plan, implement, monitor, and evaluate the options and services required to meet an individual’s health needs, using communication and available resources to promote quality, cost-effective outcomes. The care manager identifies appropriate providers and facilities throughout the continuum of services, ensuring resources are used in a timely and cost-effective manner to obtain optimum value for the patient and the reimbursement source. The role supports the increased volume and the overall care management process and department.
Responsibilities
Utilize a collaborative process to assess, plan, implement, monitor, and evaluate patient needs.
Identify appropriate providers and facilities across the continuum of services.
Promote quality, cost-effective outcomes and timely use of resources.
Support the care management process to handle increased volume within the department.
Qualifications
Education: Graduate of an accredited ASN/BSN Program in Nursing.
Licensure/Certifications: Licensed Registered Nurse in the Commonwealth of Massachusetts; CCM or related certification preferred within 24 months from hire date.
Experience: 3-5 years of diversified clinical experience; minimum 2 years in judgment/decision making, preferably in utilization management or case management.
Knowledge and Skills: Knowledge of medical/surgical practice patterns, medical terminology, managed care concepts, assessment of physical/mental impairment, confidentiality and legal/ethical considerations, ICD-9/CPT nomenclature, benefits/plan contracts, and strong interpersonal, written/verbal communication, and data management skills.
